Scholarship application declaration

I agree that I am responsible for the information lodged via a scholarship application.  I acknowledge and accept that there are serious consequences if incorrect information is provided via the scholarship application. I understand that the application I lodge will not be considered via the Melbourne Scholarships Office for scholarship until:

  • the application is complete;
  • the required documents are submitted by the application closing date.

I declare that I have read and understood the eligibility and selection criteria outlined on the Melbourne Scholarships Office (MSO) website and that the information supplied by me on this scholarship application is complete, true and correct in every particular. I acknowledge that the University may terminate any scholarship if I misrepresented my past and/or present circumstances and that this termination may take place at any stage during the course I undertake.

I am aware that I must not continue to receive more than one scholarship from different institutions, and that I must not be concurrently enrolled in another course, unless approval is granted from the appropriate University authority.

I authorise the University to obtain official records with respect to me and to seek other relevant information about me, including details of my enrolment, academic record, examination results, scholarships and awards already held, bond status, attendance and addresses during the year in connection with my application for scholarship and with regard to any benefits payable to me under the Award.

I agree to abide by the institution's conditions of award as amended from time to time. I am aware that giving false or misleading information is a serious offence under the Criminal Code (Commonwealth).

I understand that the information provided in the scholarship application will only be used by staff at the University of Melbourne with direct responsibility for scholarship selection and will not be used for any other purpose, except as required by law. I understand that I have the right to see any personal information held by the University (enquiries: privacy-officer@unimelb.edu.au).

I am aware that the Department of Education, Employment and Workplace Relations (DEEWR) may request copies of my application form and/or documentation from the Melbourne Scholarships Office.

In regard to any scholarship/s that the Melbourne Scholarships Office offers to me, I agree that the University may release relevant personal details* and my photo for publications, promotional and marketing, and reporting purposes (* eg your name; the name of the scholarship/s awarded; and name of the course for which the scholarship was awarded; brief information about your intended research/studies).

I also agree that the University may disclose relevant personal details to:
- scholarship funding bodies if the funding body requests a list of applicants and/or recipients, progress reports, etc; and
- tertiary institutions who may seek information about my application for scholarship and/or any scholarship payments I have received.
